About The Community Health Foundation

Who We Are

The Community Health Foundation is an independent, non-profit 501(c)3 organization that exists to improve the quality of life for people in Wright County.

For more than 25 years we have invested in grassroots efforts and organizations that impact health. In 2008, CHF merged with Elim Care, Inc. to reduce operational costs and expand its housing and health care services in Wright County.

The foundation is led by a volunteer Board of Directors, whose members give their time and experience to guide the work of the foundation.

The Community Health Foundation of Wright County serves as a quiet yet powerful force for wellness across the region, funding grassroots projects that touch everyday lives in meaningful ways. Through careful stewardship of donations and partnerships with local organizations, the foundation supports initiatives ranging from hospice and comfort care to community walks and educational events. Each project reflects a deep commitment to building healthier, safer, and better-informed communities, one neighbor at a time. The foundation's work is rooted in the belief that lasting change begins at the local level, where compassion meets action and where every contribution, no matter the size, helps weave a stronger fabric of care throughout Wright County.

Whether participating in an annual butterfly release, attending a fall dinner, or joining a golf fundraiser, community members find meaningful ways to connect and contribute. The foundation also offers opportunities to give through memorials and ongoing donations, ensuring that resources remain available for those who need them most. By fostering a culture of generosity and collaboration, the Community Health Foundation continues to expand its impact, creating a legacy of health and hope that will serve Wright County for generations to come.
